After services are requested, a ComForcare nurse visits the client at home to gather all pertinent medical and personal information, including a safety and fall risk assessment. Then, ComForcare partners with the client and family to create a customized care plan based on this information. The ComForcare Care Plan addresses both the physical and social needs of every client.

During the assessment, the nurse also reviews the proposed schedule of services and answers any questions the client or family may have. Services can be scheduled for short-term transition periods, or on an ongoing part-time or full-time basis. Respite services are also available to provide relief for family caregivers so they can take a break from their caregiving responsibilities, travel or attend to personal matters.
